Section 105

Your employer has purchased a high-deductible health plan with your Insurance Carrier. Your employer is self-funding the high deductible. If you or your providers contact the carrier directly you will be told that you have a high deductible health plan (which is what your employer purchased from the carrier). However, your plan is being administered by IIS Benefit Administrators. You actually have co-pays to meet and a lower deductible which is applied to: hospitalization, outpatient surgery or out-of-network benefits. (Refer to your plan design for listing).

You may receive or have received (2) ID cards. One from the carrier and another one called a Max 105/IIS Benefit Administrators ID card which is to be used for benefit eligibility purposes and pharmacy. Use your Max 105 as your medical ID card. When your provider calls the number located on the back of your Max card your actual deductible and co-pay amounts will be given by IIS Benefit Administrators.
* * * * Your Carrier I D card should not be used.
